چکیده
Background: Although ceftazidime/avibactam (CAZ/AVI) has become an important option for treating adults and children, no data or recommendations exist neonates. We report a neonatal sepsis case due to CAZ/AVI-resistant blaKPC-2-harboring Klebsiella pneumoniae carrying blaVEB-25 the use of customized active surveillance program in conjunction with enhanced infection control measures. Methods: The index was extremely premature neonate hospitalized 110 days that had been previously treated multiple antibiotics. Customized molecular implemented at hospital level measures were taken early recognition prevention outbreak. Detection identification performed using next-generation sequencing. Results: This first bloodstream caused by KPC-producing K. resistant CAZ/AVI without presence metalo-β-lactamase multiplex PCR platform neonate. All 36 additional patients tested (12 same NICU 24 from other departments) carried wild-type blaVEB-1 but they did not harbor blaVEB-25. Conclusion: emergence is signal horizontal transfer plasmids facilities it greatest concern maintaining sharp vigilance novel resistance mechanisms. Molecular diagnostics can guide appropriate antimicrobial therapy implementation against resistance.

Predictors of mortality in patients with bloodstream infection due to ceftazidime-resistant Klebsiella pneumoniae.
Bloodstream infection (BSI) due to multidrug-resistant Klebsiella is associated with high rates of morbidity and mortality. The aim of this study was to identify predictors of in-hospital mortality among patients with BSI due to ceftazidime-resistant (CAZ-R) Klebsiella pneumoniae at a tertiary care medical center. Patients with CAZ-R K. pneumoniae BSI were identified by our microbiology laborat...

Multidrug resistance associated with extended-spectrum beta-lactamase (ESBL) and Klebsiella pneumoniae carbapenemase (KPC) among K. pneumoniae is endemic in southern Europe. We retrospectively analyzed the impact of resistance on the appropriateness of empirical therapy and treatment outcomes of K. pneumoniae bloodstream infections (BSIs) during a 2-year period at a 1420-bed tertiary-care teach...

In a molecular, microbiologic, and case-control study to describe the epidemiology of ceftazidime-resistant Klebsiella pneumoniae and Escherichia coli bloodstream infection, 32 unique isolates were recovered over 31 months from the blood of patients hospitalized in a 900-bed hospital in Chicago. Multivariate analysis revealed cases occurred more frequently in debilitated nursing home patients w...

RATIONALE Carbapenem-resistant Enterobacteriaceae infections are a serious health care problem, because of the high mortality. Carbapenem resistance is mainly caused by carbapenemases production, including Klebsiella pneumoniae carbapenemase (KPC). Ceftazidime-avibactam is a new cephalosporin/β-lactamase inhibitor combination for the treatment of complicated urinary, intra-abdominal infections,...
